I hereby consent to and authorize Faith United Methodist Church the absolute right and permission to use any images, video, audio and likeness of me and/or any statements or stories from me in publications, advertising or other media activities for the sole purpose of promotion by FUMC related to our mission and our relations in the community.

This includes, but is not limited to news releases, photographs, video, audio, website, newsletters, brochures, social media, flyers or other forms of electronic or print literature.

I hereby waive any right to inspect and/or approve the finished product or advertising copy that may be used in connection therewith or the use to which it may be applied for use.
